The E.D.I.T. Protocol gives PMHNPs a systematic way to evaluate, discuss, initiate, and track a safe deprescribing plan — for the fragmented med lists, prescribing cascades, and legacy benzodiazepines that show up in your schedule without warning.

Sound familiar?
You inherit these charts. Nobody handed you a clean diagnostic picture and a rational med list — you got a history that drifted, often across multiple providers, often without anyone stepping back to ask: does this still make sense?
That question — does this still make sense? — is where you come in. And most training never gave you a systematic way to answer it.
The shift
A four-phase framework for evaluating and safely deprescribing a fragmented or outdated medication regimen — learned through four real, fully worked patient cases, not as an abstract model.
Before you touch the med list, audit it against the actual clinical picture. Does each medication still match a real diagnosis — or is it treating a side effect of another medication? This is where prescribing cascades get identified.
Deprescribing fails when framed as removal. You're proposing an optimization trial with an explicit Pause-and-Reverse contract: if anything gets worse, you course-correct together.
Execute the math — including when it isn't simple. The 10-25% hyperbolic taper rule, formulation workarounds when a capsule can't be split, and the right sequencing.
Every taper needs an active monitoring decision at every step. For a fast cascade that might be weekly; for a twelve-year benzodiazepine, a taper that responsibly takes one to two years.
The curriculum
Each case walks the full protocol — Evaluate, Discuss, Initiate, Track — end to end.
A complete reference through all four phases — documentation templates, clinical rationale, and the actual language to use in the room at each stage of the conversation.
Four prescriptions, three prescribers who've never spoken. Stimulant-driven insomnia treated with an antipsychotic, possible stimulant-induced akathisia, and a PRN benzodiazepine compensating for a side effect nobody named.
Four overlapping diagnostic labels, five medications, and a documented note that the combination itself may be causing the symptoms it was meant to treat — never acted on until now.
An extended-release antidepressant that can't be tapered by standard means, plus a blood-pressure medication and an ED prescription that may both be treating the antidepressant's side effects.
A benzodiazepine in place for over a decade, now a documented fall and cognitive risk — the case that teaches how much the human side of a taper matters alongside the pharmacological side.
