You Have the Diagnosis. Now What?

The Treatment Accuracy Blueprint gives PMHNPs a systematic, evidence-based way to turn a working diagnosis into a defensible treatment plan — what to start, what to check first, what to monitor, and how to know in four to eight weeks whether it's actually working.

9 sessions, each with a built-in case study · 3 CE credits via Pinnacle · adult-focused

Sound familiar?

There's a moment in every evaluation that training doesn't prepare you for.

You've done everything right — the intake, the history, the mental status, the differential. You have a working diagnosis. Then you sit down to write the plan, and there's this pause. What do I actually do with this?

Do I start a medication today or wait? If I start something, which one?
What do I check before I prescribe — and what am I actually monitoring at the next visit?
What does success even look like here? Not a cure — the realistic, specific sign it's working.
The note says what I prescribed, but not why. Six months later, no one can follow my reasoning.

You get taught how to diagnose — DSM criteria, the differential, the evaluation. Then there's an enormous gap between “this is what I think it is” and “here is what I'm going to do about it.” That gap is what TAB closes — not with a protocol, but with a system for thinking that makes those decisions faster, clearer, and more defensible every time.

The shift

Three questions before every prescription

If you can answer all three before you sign the prescription, you have a treatment plan. If you can't, the plan isn't ready yet. That same discipline scales into a full Treatment Planning Hierarchy for complex, multi-problem patients.

1
What am I treating?

Not the diagnosis — the specific target. The symptom or functional impairment that will actually change if this works.

2
What am I monitoring?

What you'll check, when, and what counts as a signal that something needs to change.

3
What does success look like in 4-8 weeks?

Not remission — the realistic, measurable sign that this treatment is working for this patient.

The Treatment Planning Hierarchy

Safety leads. Treat the biggest functional impairment first. Find the anchor diagnosis. Name what you're deliberately deferring, in the chart, with rationale.

The curriculum

What's inside TAB

Every module carries a built-in case study — treatment reasoning applied, not just described.

Intro

Build confidence in medication management

The gap between diagnosis and treatment, how supplements and labs integrate into clinical decisions, and your quick win: the three questions to ask before every prescription.

01

From Diagnosis to Treatment: Building the Plan

Naming the target before the medication, the treatment planning hierarchy, sequencing, the expectation-setting conversation, the monitoring framework, and a treatment note that shows your thinking.

02

Treating ADHD, Anxiety, and Their Overlap

Stimulant selection, non-stimulant options, sequencing ADHD vs. anxiety, buspirone's underused role, and the labs that matter — ferritin, thyroid, cardiovascular.

03

Treating Depression: Unipolar, Mixed Features & Bipolar

SSRI/SNRI selection, augmentation before “treatment-resistant,” when to pause an antidepressant, mood stabilizers in bipolar depression, and where ketamine/esketamine fit.

04

Personality Patterns, Trauma & Autism-Adjacent Presentations

What medication can and can't do, symptom-targeted and trauma-informed prescribing, prazosin for nightmares, autism and medication sensitivity, and the right psychotherapy referral.

05

Treating SUD Alongside ADHD and Anxiety

MAT overview and scope judgment, treating ADHD in recovery, the benzodiazepine conversation, alcohol-use pharmacotherapy, and the labs that protect patient and license.

06

Preventing & Managing Iatrogenic Complications

Akathisia, antidepressant discontinuation vs. relapse, metabolic monitoring, medication-induced mania, emotional blunting and sexual side effects, and real deprescribing conversations.

07

Complexity, Supervision, Collaboration & Emerging Treatments

When to consult vs. co-manage, the reproductive-psychiatry pivot, TRD referral pathways (TMS, ECT, esketamine), and honest conversations about psilocybin and MDMA-assisted therapy.

08

Building Your Treatment System

CLARITY as a treatment-note structure, the one-paragraph formulation, informed consent, monitoring schedules by class, and prior-auth notes reviewers actually approve.

9 sessions (Introduction + 8 modules), each with a built-in case study · 3 CE credits via Pinnacle — jointly accredited ACCME, ACPE, ANCC.

Is this you?

  • You want documentation that holds up to a colleague, patient, or licensing board six months later

This isn't for you if…

  • You want a medication-by-medication formulary walkthrough rather than treatment selection and sequencing
  • You're looking for something to replace clinical judgment rather than sharpen it

Questions

Frequently asked

Do I need to take DAB first?
No — TAB stands completely on its own. If you have taken DAB, you'll recognize CLARITY as shared clinical language; TAB picks up exactly where diagnosis leaves off.
Is this eligible for CE credit in my state?
CE credit is provided via Pinnacle, jointly accredited by ACCME, ACPE, and ANCC. Confirm your state board accepts these accreditors.
Does this cover child/adolescent prescribing?
TAB is adult-focused, consistent with DAB. It centers on the presentations that fill outpatient practice.
Does this replace consultation with a collaborating physician or psychiatrist?
No — TAB makes your treatment decisions faster and more defensible. It complements collaboration and supervision, never replaces them.
